No, it is the same process. Our system sends an automated invitation to your email to help you prepare for your upcoming visit. If you have already completed your registration through the link provided in that email, you do not need to use the online or print options found on this page. Both methods securely transmit your information directly to our clinical team at Downriver Surgery Center.
Our patient portal is fully responsive, meaning you can easily and securely complete all necessary forms directly from your smartphone, tablet, or desktop computer. If you prefer pen and paper, you can download the printable PDF versions here or arrive 15 minutes early to your appointment to complete them in our office.
When you select the online option, you will be prompted for your email and password. If you have not created a password or have forgotten it, simply click "Forgot Password" and follow the on-screen instructions. To ensure a seamless, zero-wait check-in process, we ask that all new patient and medical history forms be completed at least 24 hours prior to your scheduled consultation or surgery.
Absolutely. Your privacy is our top priority. Our digital intake forms utilize state-of-the-art encryption and are 100% HIPAA-compliant, ensuring your personal and medical data is securely transmitted directly to our clinical system.
If you are unsure about a specific diagnosis, medication, or past surgery, simply leave that section blank or add a note in the comments. Our clinical team will review your history with you in person during your consultation to clarify any missing details.
If your referring dentist has taken panoramic X-rays or a CBCT scan within the last six months, please request that they email the digital files directly to our office prior to your consultation. If they are unable to send them, we can take updated, high-definition 3D imaging during your visit.
